Teeth Cleaning Consent Form

I hereby give consent from this day forward to staff from Precious Paws to carry out teeth cleaning on my dog.

Payment and Cancellations

• Cancellations must be made with a minimum 24 hours notice.  Cancellations made later than this will be subject to a cancellation fee of 50% of the groom.

• Should nobody be available to handover the dog (i.e. if nobody is home) at the given appointment time, the cost of the groom will be charged and any further bookings will require payment 48 hours prior to the appointment.  Failure to do so will result in the appointment being cancelled.

• Payments can be made by card, cash or bank transfer.  Payments must be made within 24 hours of the groom.  Failure to do so will incur interest at the rate of £2 per day and may result in legal proceedings.  Future bookings will require payment 48 hours prior to the grooming appointment, otherwise the appointment will be cancelled.

I understand that:

• Staff will be using the Cleany Teeth toothbrush on my dog.

• Staff will be taking before and after pictures of my dog’s teeth.

• Tartar and plaque may be removed from the teeth during the cleaning process and that a non-sharp tool may be used to aid this process.

I understand that, should any of the following occur, the session will need to be stopped and that I may need to seek veterinary assistance:

• Any loose teeth are suspected.

• If my dog objects and becomes stressed or aggressive.

• Any obvious abnormalities are visible or suspected within the mouth such as oral warts, embedded foreign bodies, injuries or tumours for example.

• Veterinary costs will be paid by myself or through my insurance.

I understand that:

• There may be minor reddening or minor signs of blood on the gumline during and after teeth cleaning as the plaque is removed.

• If a session, once started, has to stop for any health or behaviour reasons, that I am still liable for the full cost of the session and agree to pay this upon picking up my dog.

I declare that:

• I do not know of any medical reasons as to why teeth cleaning cannot be carried out on my dog.

• I acknowledge that it is my responsibility to inform staff of any changes to my dog’g oral and general health at each subsequent session.

• I am aware that the teeth cleaning service is not a substitute for a veterinary inspection and that staff are not qualified to diagnose abnormalities or disease.

• I will not hold Precious Paws liable for any pre-existing dental issues included, but not limited to, decay, gum disease, loss of enamel.

I consent to all of the above and confirm that I am the legal owner of the dog.
